The situation

What has happened

Harriet Lowe, a learner in the blended Certificate IV program, is ready for her observed delivery assessment at her practicum host, a residential aged care provider. The current assessment tool requires an observation of a 40-minute session, a post-observation interview and a review of her session plan and learner feedback. Her support plan records extra time for written tasks. The Lead Assessor expects the assessment to be planned, briefed, conducted and recorded exactly as the Assessment Conduct Procedure requires, with the outcome in the student register within ten business days and feedback the learner can act on.

What to look for

Evidence guide

Check that the plan and briefing cover every required element and that the adjustment is applied and noted. The observation checklist must be completed as evidence is seen, not afterwards. The decision must be traceable to the evidence and the rules of evidence, and the feedback must be specific. Recording within the timeframe and stating appeal rights are essential.

The student's evidence summary lists every record they created or changed in the systems named above, their notes and the tasks they ticked. Verify it against the deliverables and your own assessment tool.
