Assessment Appeal Form

Purpose. Learners use this form to appeal an assessment decision, and Student Services uses it to register the appeal and start the independent review.
1.When to use this form
Use this form within 20 business days of receiving an assessment result you believe is wrong or was reached unfairly. You may talk to your assessor first, but you do not have to. Lodging an appeal is free and will not affect how you are treated.
2.What to include
State the unit, the task, the date of the decision and the assessor. Explain why you believe the decision is wrong, for example evidence that was not considered, a reasonable adjustment that was not applied or a marking guide applied inconsistently. Attach any further evidence you want considered.
3.What happens next
Student Services will acknowledge your appeal within five business days and enter it in the complaints and appeals register. An independent assessor will review the original evidence and any new evidence and may ask to meet with you. You will receive a written outcome with reasons within 30 calendar days.
4.Support
You may bring a support person to any meeting. If you need help completing this form, contact Student Services or the language, literacy and numeracy specialist.
5.If you are not satisfied
You may request an external review by an independent mediator arranged by the Institute, and you may contact the national regulator at any time.
