Purpose. Learners, trainers or the specialist use this form to request an adjustment to training or assessment arrangements for an individual learner.

1.When to use this form

Use this form when a learner needs a change to how training is delivered or how assessment is conducted because of a disability, a health condition, a language, literacy, numeracy or digital skills need, or a personal circumstance. Requests can be made at any time and are best made before assessment.

2.Who completes it

The learner completes the form with help from Student Services if needed, or a trainer completes it on the learner's behalf with the learner's agreement. The specialist completes the assessment and recommendation section.

3.Evidence

Supporting documents such as a medical or allied health report, a previous education support plan or the foundation skills assessment result help the Institute decide quickly, but a learner will not be refused a conversation because they lack documents.

4.Decision

The Student Services Manager will approve, vary or decline the request in writing within ten business days and record the outcome in the learner's support plan. A declined request may be appealed under the Complaints and Appeals Policy.

5.Privacy

Information on this form is used only to arrange support and is shared only with the trainers and assessors who need it.
